Background Information

You will be asked to fill out our New Patient Form giving us pertinent information about your medical and dental history. This information helps us to provide you with the best possible care. If you would like to fill our your paperwork ahead of time, click on adult form or child form. 

 

Comprehensive Exam

Your comprehensive exam will consist of an oral cancer screening, full mouth digital x-rays, and a thorough dental exam. 

 

Treatment Plan

Our highly trained staff will then use what they've learned from your background information, your input, and the comprehensive exam to construct our recommended treatment plan. We are happy to answer questions you may have regarding your treatment plan at any time. 

 

Financial Consult

Our office staff will be happy to answer any questions you may have about financing your future dental procedures.

 

Schedule Future Appointments

Finally, you will work with our front desk staff to schedule any needed future appointments for your treatment plan.
